Copyright Transfer Agreement 求助

本人第一次中了篇SCI文章，没有任何经验，生怕出错，所以想求助好心的朋友们。请问院子里面的朋友，Copyright Transfer Agreement 怎么签呢？

Date:（这个容易些）

Contributor name: （所有作者都需要签上名字么？）

Contributor address:

Manuscript number (if known):  （签投稿时候的ID号，还是修稿后最总被接受的ID号啊？）

Re: Manuscript entitled(the “Contribution”) （这个是什么意思呢？，还有个“Re”）

for publication in  (the “Journal”)  （这个签投稿的杂志吧？）

published by   (“Wiley-Blackwell”).（这个是不是把“Wiley-Blackwell”手写一遍就行啊？）

除此之外还有一个
Form for collecting details of authorship

Manuscript number……………………………….(issued when you complete submission)





这个表格也让填写，实在后面打勾就行么？还是怎么填写呢？希望有经验的朋友帮助，非常感谢啊！！！！！！！！！！！！！！
