I am not aware of a more comprehensive text dealing with the micro- and macroeconomics of the US health care sector (but then again, I haven't read through many health econ textbooks). However be advised, this book was published May 28, 2009, 5 days after sweeping health legislation (PPACA, and later HCERA) was passed, so it does not contain the most up-to-date information on the effects of reform on health economics, but provides a sufficient amount of detail and speculation to fill in many of the gaps. The text is very straightforward (although there's not much you can do to make health econ sound particularly exciting).
The online material was of no use -- I tried to gain access to it but the website is plagued with problems. This was not a big deal as I never use online resources included with textbooks, and probably wouldn't have used this one anyway.
I was inclined to rate 4 stars because the price is so exorbitant, but there aren't many books on the subject that are this comprehensive -- guess they have a MONOPOLY on health econ textbooks. So the 5 stars represents my opinion of the quality/content of the book, separate from its unfortunate price.
