博士研究生个人培养计划表

说明：
1． 本表由博士生导师和博士生共同填写，一式三份，学院、导
师和博士生各一份。
2．本计划表应于入学后三个月内完成，由各院收齐、审查。
3．博士生毕业时各学院须将一份完整的表格存入本人档案。
4．本表须认真填写，内容简明扼要。
Instruction:
1. The form shall be filled by both the doctoral student and the
supervisor in triplicate, each for the department, supervisor and the
student.
2. The form shall be filled within three months after entering school,
and shall be collected and reviewed by the department.
3. The department shall save a filled
form with the files of the student
at graduation.
4. The form shall be filled in concise but explicit words.
